Out of curiosity, what does this do differently to the native org-mode bookmarking? I use that quite a lot and have never had issues.

I agree, it’s not clear what’s more, probably the README should clarify it better.
I guess, reading it, that maybe (that’s what’s not so clear) the library creates unique IDs when bookmarking a Title element?
But again that’s something you can do manually or automate natively without the library and in any case the behavior of the built in bookmarks doesn’t change, AFAIK.
What are we missing?

I believe it’s for bookmarking org *headings*. Not just org files. I’m not sure you can do that with the standard bookmark capabilities.

That's correct. By default, Org bookmarks can only bookmark files, not headings within them. Obviously, that's not very useful, since a file may contain many thousands of headings, and their position may change regularly as the file is edited.
