i have the following code in my org mode document
#+begin_src latex :file test.png :results file graphics :wrap center
x = 20
#+end_src
everytime i type `C-c C-C` in order to execute the code block i get another block of output so i end up with multiple blocks like:
#+RESULTS:
#+begin_center
[[file:test.png]]
#+end_center
#+begin_center
[[file:test.png]]
#+end_center
#+begin_center
[[file:test.png]]
#+end_center
however if i remove the `:wrap center` it works just fine, how can i prevent this?

That does sound strange. Could it be from an inherited source block header property defined elsewhere in the file? (Can't think how that would work, but might be worth checking).

even if i opened a blank org file and inserted this i would get the same result so i guess its a bug in org mode?

Try `:results output replace`

tried it didnt work, anyway im already using the :results keyword im not sure i can use it twice

Yeah, it seems to work with `:wrap <anything-except-center>`, so this is strange indeed. I also notice that with `:wrap center` the output does not seem to be a part of the `#+RESULTS:` drawer since it does not collapse when you tab on it, though it does with all wraps other than `:wrap center`.
Might be a good thing to send along to the org mailing list.

FYI, I am not able to reproduce using Org 9.5.3
