It also doesn't recognize a second level even if I type it in. I think the relevant lines in init.el are
;; When ‘buffer-name’, use the buffer name.
(setq org-refile-use-outline-path t)
;; using ido (or other completion package), so i don't need to complete in
;; steps
(setq org-outline-path-complete-in-steps nil)
;; When refiling, set new items to be put as the first child instead of last
;; nil = last, non-nil = first
(setq org-reverse-note-order t)
Here's my beginner-level init.el file, if needed:

What is your question? This is how it works for me too. Do you have reason to believe it should be otherwise?

I was trying to refile to a subheading, not top level. I now see that I need to set up `org-refile-targets`. I was confused because the emacs help doc for `org-refile-use-outline-path` sort of implies (to a newbie) that setting would allow one to refile to a third level subheading.
So I hacked this from an example:
(setq org-refile-targets '((nil :maxlevel . 9))
I also had to set `(setq org-outline-path-complete-in-steps nil)` back to `t` since ido-mode had too many completions to choose from. Working now!

Nice - it's always good to see someone help themselves. The docs are so extensive that I find myself getting lost in them. Better than the alternatives, I guess.
