Is it possible to alter my consciousness in such a way that I can enter the body of my younger self with all of my personality and knowledge from my present self and live from that point on? Do you know anyone who has done this and could it ever be possible at some point?

Haha. This universe must be a joke at this point but alright, the more you know... 1st of all, where did you got this information ? Did you found it on the wired or you connected the dots yourself ? If you found it on the wired then mb, there's a possibility it come from me, I have a very strong metaphorization in the wired and I left a though about a plan where a such possibility exist. But I never tried it and I don't even know if it's possible. It was only an over estimation of a collection of possibilities that can lead to this possibility, It was only a theory / a possibility, and it never got tried at my knowledge. If it's possible that would be probably the worst experience any living been could ever do, you would basically become a ''god'' because of the immortality this would give. And what's the point of going back in time for changing something in a ''world / universe / dimension'' that isn't ''ours'' originally ? Also it's very easy to be addicted to reality and to abuse a such thing. I could continue deeper, but It's better to stop here. It's just a theory I left in the wired, I use this theory for some ''demonstration'' to someone > I needed to make him believe it was possible for demonstrate something else, so I searched all real possibility to make something like that, and collected all the most realist possibilities. But for create such ''device'' it would require anyway knowledge you probably don't have and will never have unless you scarify you life to the research of advanced information which that require inhuman conditions to access. And if you found that by connecting the dots alone then, you have a really power of parallel computing, but if you cannot connect the dots to go farer and create the plan by yourself then don't think about it, the implications behind the understanding and the creating of such plan need a mind able to survive high speed of movements. I can at least tell you this. If you can reach such speed in the wired you may find thing way bigger than this file. Just know that the ''conditions'' to reach such speed can lead to death if not understood and applied correctly. Also you will to do a such thing may be resolved by simpler way in this world than going back in your younger self.

Of course it's possible. Not in the classical back to the Future type of way, because time is not a chain, it's a tree, multi-dimensional one at that. But of course it's possible. Much Wilder things than that as possible, sir. Don't forget, we live in the world where Infinity exists. You don't have the imagination to comprehend how wild, how spacious, how just insane... Anyway, going back to your original question. The real question is not whether or not it's possible, but whether or not it's relevant. And the truth is that what the ego thinks is relevant for it can often be quite a ways apart from what the soul thought was relevant choosing to live this particular life. I'm not saying it's not relevant for you, but I'm saying it's probably highly unlikely that it's relevant for you and if it's not relevant to the themes you came here to explore, it's simply not going to happen. Because the ego cannot really Force the flow of your soul. In other words, if it would require it for you to go against the grain, it doesn't matter what you do, it will not happen. If it's in your cards so to speak, then it's almost irrelevant what you do as long as you're being yourself - it's just going to happen. For example back in the native American days, there was a guy who just exchanged bodies with another guy and both went on living. Just exchanged consciousnesses. It would certainly be entertaining seeing a Mantis or some insect de molt itself and shove itself back into its exo like nothing has happened.
