I'm interested in creating new banners. What are the restrictions on image size and file size/type?

I've saved several threads and happen to have several not yet posted by others. Does anyone have the John McCarthy banner? That's a very old one. I remember when I recommended it, but didn't do it well and someone else did better for me.

Planning on making more creative ones in the future. For now, I really want to see this as one. :D

Are banners automatically added from this thread? or do they need to be approved. Anyways, here's mine. (reupload because last time the file was 3M+) I have different colored variations if anyone wants to see.
