Why would you even need a manual? Why would you do it in such a deeply unsettling and perverse way as building that bizarro thing in the picture? Shit's stupid as fuarrrk, and if that's part of your criteria and value system, chances are you don't need a manual.
Anyway, all you really need to do is stop drinking water or eating. Go lay out in the woods or something, wait for the deep stupor and elements to take you. Ah... but you cannot. And you will not. Why? Doubt. You cannot overcome your life instinct function. Which means that you must continue.
I know (of) a woman in her 70's that had two strokes, her kids were after her money, she was tired of the way the world was going perhaps, and just decided to she was ready to go. Got a state whatever it is (legal matter for later stuff) to hang around, friends and neighbors came by and hung out. They said she seemed very peaceful and that it was a strange situation in general. It only took around a week. She slipped into a state resembling sleep, then gradually faded away.
I don't know what you're facing Lain, and I've lived a pretty dark decade myself, but this isn't the way. I'm also not sure about a case for morbid curiosity either. It looks pretty retarded imo, but that's me.

that's stupid and undignified, having to resort to dehydration and starvation. You'll feel hunger and thirst, they hurt. As you said, "you cannot overcome your life instinct function", but that applies more to the need to eat rather than having the will to die and going through with it. You might wait and stall, but if it is truly decided the free will is stronger than your body struggling with the idea.
Also waiting for your body to shut down eventually is not a nice death as those waiting for it in hospices will tell you. Aside from hunger and thirst not knowing when it'll happen is truly an awful idea. If I'd kill myself I'd want to have control over that, not being subject to running low on energy.
While op didn't ask for instructional advice and might just be curious about the book, I've always wanted to go with a "suicide mask", as it gives control and let's you drift off peacefully. It's a shame good helium is too expensive for me and I don't want to resort to cheapass helium for balloons. Even though it would be quite funny to kill myself with something that's 1. for parties, and 2. has a happy clown on it.

>anything is better than death
Lack of imagination. There are many things worse than death, unless you think that we all go to hell when we die.
